Need to make changes to your personal information? Recently move? Have a beneficiary change? Here's what you need to know:

You will need to update any ADDRESS CHANGES/NAME CHANGES in the following places:

• Human Resources - Send an email to [email protected]
  • Fire Employees - Send an email to [email protected] 
    • GA Firefighter Pension Fund - Fill out THIS form then either email or fax (information listed at the top of the form)
• Medical/Dental/Vision Insurance - Log into Plansource and toward the top right section of the page, you will see an option to update personal information.  
  • Your password is the last 4 digits of your SSN followed by the last 2 digits of your birth year
• Defined Benefit Plan/ACCG - Address changes are fed to ACCG bi-weekly via payroll processing;(Active employees) as long as you have changed your address with Human Resources, your address with be updated with ACCG.
• VOYA -  view/make changes.
• Colonial - Call the customer service line (800) 325-4368 to make changes.

You will need to update any BENEFICIARY CHANGES in the following places:

• Defined Benefit Plan/ACCG - ACCG Portal
• VOYA - view/make changes. 
  
• Colonial - Call the customer service line (800) 325-4368 to make changes.
• Prudential - Log into Plansource and toward the top right section of the page, you will see an option to change your beneficiary
• GA Firefighter Pension Fund (Fire Dept. Employees ONLY) - Fill out THIS form and either email or fax (information listed at the top of the form)
