Lab Collection hours for DUI Court participants are 6:00am-10:00am, 7 days a week.

WEBSITE INSTRUCTIONS (PREFFERED METHOD)

1. Visit https://verify.phoenixlis.com/ daily 12:01 AM - 9:59 AM. YOU MUST CHECK IN 7 DAYS PER WEEK, 365 DAYS PER YEAR, INCLUDING WEEKENDS AND HOLIDAYS.

1. Enter the ACCOUNT# LVWTOV8X

1. Enter your ID#: ____________________________ (2-digit birth month, 2-digit birth date and last five numbers of social security number or assigned ID)

1. Press “Submit”

1. The screen will display one of the messages below with a confirmation code:
• REPORT FOR TESTING.
• NO TESTING TODAY.

           **If you receive ERROR 432: Lab Not Found, please verify the account # is correct and re-submit.  If you receive any other response during the 12:00 a.m. to 10:00 a.m. time frame, please contact DUI Court ASAP.**
